What is a telephone consultation?

A Telephone consultation is where the clinician can discuss your problem with you. You may be sent a text message link to attach photos of your problem before the telephone consultation this is so the clinician can view these images whilst discussing with you. After gaining a background of knowledge the clinician may decide that they need to see you in person and will arrange and book you in for a face to face appointment

What is a online consultation via video call?

An online video consultation is where the clinician can discuss your problem with you. You will be sent a link to join the video call prior to your appointment time. After gaining a background of knowledge the clinician may decide that they need to see you in person and will arrange and book you in for a face to face appointment

What is a Face to Face consultation?

A Face to Face appointment is for problems that cannot be discussed over the phone or online as a physical examination needs to go ahead

What is a home visit?

If you require a home visit please try and contact the surgery before 11am and alert the reception team giving a brief description of the symptoms ongoing. The reception team will then alert the GP that a home visit is required. The GP will then review and arrange for a home visit whether it be with themselves or another service who may be able to see the patient.

Have you been off work and you're now wondering what happens next?

You do not require a doctor's sicknote for any illness lasting seven days or less
If you are sick for seven days or more you require a Doctors sick note
If you feel okay to go back to work but cannot do your job to the full extent you may need a 'Fit Note'

How do I self Cert for 7 days?

Your Employer may require you to complete a self-certification form (SC2) which will be available via the company or the government website HMRC Self Certification Form

How do I get a sick note?

Once you have gone over the time period of 7 days you will require a Doctors sick note.

Please contact the surgery on 01902 722359 and ask the reception team for a sick note.

You will then be sent a text message to respond to please include the following in the response:

The day your self certification ended
A brief description of your current situation
The time frame you are requesting off

Please kindly fill out this text message as soon as possible otherwise this will cause delays in creating the sicknote for you.

Once we have received a response this is added onto the next available GP query for them to action as soon as this is done you will receive a message back with your sick note attached.

What if I don't have a smart phone to do this on?

Please do not worry if you do not have a smart phone as the team can work in other ways to arrange all the above to happen for you

Statement of fitness for work - 'Fit Note'

The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury.
