 What is a telephone consultation?

A Telephone consultation is where the clinician can discuss your problem with you. You may be sent a text message link to attach photos of your problem before the telephone consultation this is so the clinician can view these images whilst discussing with you. After gaining a background of knowledge the clinician may decide that they need to see you in person and will arrange and book you in for a face-to-face appointment

What is an online consultation via video call?

An online video consultation is where the clinician can discuss your problem with you. You will be sent a link to join the video call prior to your appointment time. After gaining a background of knowledge the clinician may decide that they need to see you in person and
will arrange and book you in for a face-to-face appointment 

What is a Face-to-Face consultation?

A Face-to-Face appointment is for problems that cannot be discussed over the phone or online as a physical examination needs to go ahead 

What is a home visit?

If you require a home visit, please try, and contact the surgery before 11am and alert the reception team giving a brief description of the symptoms ongoing. The reception team will then alert the GP that a home visit is required. The GP will then review and arrange for a home visit whether it be with themselves or another service who may be able to see the patient.

Urgent Telephone Triage / Urgent Face to Face appointment with a GP

If you have a new short condition that requires urgent treatment

• Unwell child with persistent fever, D&V or rash

• Concern over child (especially babies/infants) 

• Children with abdominal pain

• Acute severe pain (not responding to pain killers)

• Pregnancy – vomiting, pain or bleeding

• Insulin dependent diabetics

• Asthmatics – worsening symptoms

• Heart palpitations (no chest pains)

• Severe new headache

• Acute (new onset) rash

• Testicular lump/pain

• Suicidal ideation

• Severe depression (double OPD)

• Acute severe back pain

• Patient with extreme concern

A&E / urgent care centre

• Head injury
• Fracture (broken bone) sprain/acute injury with loss of function
• New traumatic injuries (within 72 hours) that are affecting the ability to carry out tasks
• Nosebleed for longer than 20minutes
• Major burns/scalds
• Poisoning (unless unconscious =999)
• Eye injuries, especially vision problems

• Chest pain (>40)
• Difficulty breathing
• Unconscious/difficulty in rousing
• Stroke/sudden vision loss/ speech difficulty (new symptoms)
• Fitting (longer than 5 minutes in epilepsy)
• Vomiting blood
• Ill child (floppy/drowsy)
• Severe bleeding
• Spinal injury
• Severe allergic reaction, especially if lips/tongue swelling

Mental Health Practitioner

Mental Health Practitioners are experienced mental health staff with expertise in the assessment and management of mental health conditions.
The role of the Mental Health Practitioner is to assess people’s needs, provide advice and guidance and support access to appropriate services if required.

What criteria do i need to meet?

• 18 and over
• Not already open to mental health services
• Anxiety
• Stress
• Low mood

First Contact Physio

What is a First Contact Physiotherapist?
A First Contact Physiotherapist also known as a FCP, is a highly skilled clinician with expertise in assessment and decision making to manage any muscle or joint problems.

Why should a patient see a First Contact Physiotherapist instead of a GP?
To improve access to the right person at the right time.
A patient can have an expert opinion about their joint or muscle concern without a lengthy wait.

Can a First Contact Physiotherapist request investigations such as x-rays and blood tests?
Many FCPs are qualified to order investigations, but some may not be able to. However, if an investigation is required the FCP will be able to speak with a GP to get the necessary investigations.

What health issues do the FCP's deal with?

• All soft tissue injuries, sprains, strains or sports injuries
• Arthritis – any joint
• Possible problems with muscles, ligaments, tendons or bone, e.g. tennis elbow, carpal tunnel syndrome, ankle sprains
• Spinal pain including lower back pain, mid-back pain and neck pain
• Spinal-related pain in arms or legs, including nerve symptoms, e.g. pins and needles or numbness
• Changes to walking
• Foot or ankle pain
• Knee Pain / Knee Swelling / Knee Stiffness
• Elbow Pain
• Wrist or Hand Pain / Wrist Stiffness / Pins and Needles in hands
• Shoulder Pain / Shoulder Stiffness / Restricted Shoulder Movements
• Neck Pain / Neck Stiffness / Restricted Neck Movements
• Back Pain
• Hip Pain

Have you been off work and you're now wondering what happens next?

• You do not require a doctor's sicknote for any illness lasting seven days or less
• If you are sick for seven days or more you require a Doctors sick note
• If you feel okay to go back to work but cannot do your job to the full extent you may need a 'Fit Note'

How do I self Cert for 7 days?

Your Employer may require you to complete a self-certification form (SC2) which will be available via the company or the government website HMRC Self Certification Form

How do I get a sick note?

Once you have gone over the time period of 7 days you will require a Doctors sick note.

Please contact the surgery on 01902 722359 and ask the reception team for a sick note.

You will then be sent a text message to respond to please include the following in the response:

• The day your self certification ended
• A brief description of your current situation 
• The time frame you are requesting off 

Please kindly fill out this text message as soon as possible otherwise this will cause delays in creating the sicknote for you.

Once we have received a response this is added onto the next available GP query for them to action as soon as this is done you will receive a message back with your sick note attached.

What if I don't have a smart phone to do this on?

Please do not worry if you do not have a smart phone as the team can work in other ways to arrange all the above to happen for you

Statement of fitness for work - 'Fit Note'

The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury.
